2. Please complete the provided questionnaire with accurate information about your facility, food safety practices, and compliance details TMI FSVP Foreign Supplier Verification Form in its entirety.

3. Upload a valid third-party certification such as:

• - HACCP
• - GFSI (e.g., BRC, SQF, IFS, FSSC 22000, etc.)

-- Note: Ensure the certificate is up-to-date and clearly displays the certification body, scope, and expiration date.

4. Full Audit Report - Provide your most recent full audit report. Include:

• - Summary of the audit
• - Details of any nonconformance issues
• - Corrective actions taken (if applicable).

5. Product Specifications - Submit a detailed specification sheet for each product, including:

• - Product Name
• - Ingredients List
• - Nutritional Information
• - Shelf Life
• - Storage Instructions
• - Packaging Details (e.g., case pack, weight, dimensions).

Format: PDF or Excel file.

6. Processing Flow Chart(s) - Upload a processing flow chart for each product, showing all key steps from raw material intake to final product packaging.

• - If multiple products are manufactured, submit separate flow charts for each.

7. FDA Registration Verification - Upload official verification of your FDA registration. Acceptable documents include:

• - A printout from the FDA Registration Portal.
• - A certificate from a third-party confirming registration.
• - Any other official document verifying the registration.

8. Letter of Guarantee - Upload a Letter of Guarantee that ensures compliance with food safety and quality requirements.

• - Suppliers can use the provided template or submit their own version, as long as it meets the requirements.
